Residential Care Facility
Residential care facilities offer housing for individuals who need assistance with personal care or medical needs. This means that the facility is normally state licensed and meets minimum staffing requirements. The facility is staffed 24 hours a day. To be eligible for residential care facilities, an individual usually must be mentally alert; able to dress, feed, and take themselves to the toilet; able to eat meals in a central dining room; and need no assistance with personal care or behavior supervision. Additional services include meals, social activities, laundry, and housekeeping services.
Country Club Care Center offers both short-term and long-term skilled nursing care provided by our fully licensed and compassionate nursing staff that is dedicated to meeting the day-to-day needs of our residents. Our nurses are specially trained in wound care and are IV certified at every facility. We commit ourselves to helping residents achieve the highest level of physical and mental well-being possible, while treating them like members of our own family. Our goal is to always put residents first.

Assessment of All Residents
We are mindful that moving to a new place may not always be easy. For this reason, we are proud to serve our residents with a full-time admissions coordinator. Our admissions coordinator visits with discharge planners, case managers, individuals and family members who are considering either a short or a long term stay at Country Club Care Center.
Residents can learn new skills, join an exercise class, play cards or bingo, sing karaoke or participate in discussion groups. Special conversation areas are available where residents can enjoy more quiet moments with fellow residents and visitors. Residents and visitors alike can also enjoy a robust schedule of musical entertainment, movies and social activities, as well as leisurely walks in the enclosed courtyard with community garden and outdoor gazebo.
Country Club Care Center offers a wide array of freshly prepared selections from an à la carte menu. All meals can be enjoyed in the facility’s dining rooms or cozy cordial areas. The diets are designed and monitored to meet the unique nutritional and dietary needs of each resident, under the supervision of our Consulting Dietitian.
