In 1996, Capital Health Management opened Crown Care Center in Harrisonville, Missouri and Country Club Care Center in Warrensburg, Missouri. From the beginning, CHM was dedicated to providing exemplary long-term care with an emphasis on creating a home-like atmosphere where all of our residents would be cared for and comfortable. In 1999, CHM opened Wilshire at Lakewood in Lee’s Summit, Missouri, continuing our mission of personalized care rehabilitation services based on individual needs and physician orders in the Kansas City area. Services include physical, occupational, speech-language and restorative therapy. Whether an individual is in need of therapy on a short term basis or for an extended stay, our certified staff will ensure that residents regain and retain their highest possible level of function and mobility.
In 2017, CHM opened Cross Creek at Lee’s Summit Assisted Living | Memory Care, CHM’s first community that specializes in caring for those with Alzheimer’s, dementia and other memory loss issues.
Each community managed by Capital Health Management, Inc. offers personalized care and services based on individual needs and physician orders.
Capital Health Management’s care centers are designed to create pleasant surroundings with comfort in mind. Each features a comfortable interior design, beautiful landscaping and imaginative, on-site amenities. In addition, recreational and social activities offer residents the opportunity to meet new people, develop new skills and pursue hobbies and interests. The small resident gathering areas also help promote socialization in these communities.
Our Country Club Care Center, Crown Care Center and Wilshire at Lakewood Care Center communities all offer cutting edge, specialized wound care services. Capital Health Management’s newest community, Cross Creek at Lee’s Summit, offers dementia and memory care.
Education and Corporate Compliance
In order to exceed the standards of our industry, Capital Health Management encourages a culture that embraces responsibility, accountability and emphasizes continued education and improvement. Each care center is assigned a corporate compliance staff member who ensures continual compliance with all federal, state and local laws. The staff also assists in meeting our legal, ethical and professional responsibilities. We are dedicated to the development of skills and knowledge of personnel in all facilities.
Our flagship program for staff development, the Preceptor Program, makes continuing education a priority at all of our communities.
